Job Description: We are looking for a qualified Social Worker with extensive experience in working with special needs patients. The successful candidate will play a crucial role in supporting individuals and their families, developing care plans, and coordinating with various healthcare professionals to ensure comprehensive care.
Conduct assessments of patients needs and develop individualized care plans.
- Provide counseling and support to patients and their families.
Coordinate with healthcare providers, educators, and community resources to deliver integrated care.- Advocate for patients rights and access to necessary services and resources.
Maintain detailed records and documentation of patient progress and care plans.
- Participate in multidisciplinary team meetings to discuss patient care and progress.
* Provide crisis intervention and support during emergencies.
